Adult Case Management Program
Maine Behavioral Heath Organization provides case management services to persons with prolonged and severe mental heath issues to help them integrate into their communities and improve their quality of life. These services are provided with flexibility and on an as-needed basis.
The Case Manager and Client (or Guardian) will work together to identify needs and set goals that are Client focused and determined.
The Case Manager will coordinate referrals and advocate for access to resources as well as monitor service provision and regularly review progress made toward achieving goals.
Case Management Services are available to individuals residing in the Skowhegan, Waterville, Augusta, Farmington, Lewiston, and Rumford areas.
